Most Common Deficiencies For AAAHC Chapter 13 “Diagnostic and Other Imaging Services”

The most common deficiencies our surveyors see for Chapter 13 “Diagnostic and Other Imaging Services” of the AAAHC Accreditation Handbook for Ambulatory Health Care:

  • Providers must have approved privileges for each type of imaging equipment they operate.

  • Radiation safety training must be completed for all providers and staff who will be present in a room where radiation is used.

  • In addition to regular professional preventive maintenance, annual documented inspection of x-ray equipment by a radiation physicist must be present.

  • Post warning signs that alert pregnant females – in the waiting room and pre-op areas.

  • Post a radiation warning sign on the door to OR/procedure room.

  • If dosimeter badges are not utilized, have a letter from the equipment manufacturer explaining why badges are not necessary.

  • Lead aprons should be examined by radiation staff annually for holes or cracks.

  • Providers who use ultrasound equipment must be privileged, and the equipment must show biomedical inspection.
