The most common deficiencies our surveyors see for Chapter 14-I “Dental Services” of the AAAHC Accreditation Handbook for Ambulatory Health Care:

  • The full scope of dental services has not been officially approved by the Governing Body and has not been reviewed or updated periodically.

  • Privileges granted to each provider are not granted for a specific period of time.

  • Lack of a signed informed consent form for general services, such as diagnostic and preventive services or routine restorative services.

  • Initial physical exam does not include a full periodontal assessment.

  • No written guidelines for use of diagnostic radiographs.

  • No written policy on the management of pain.

  • No written policy regarding staffing requirements.
